Notice of Office Closure Announcement

It was a pleasure to care for and serve all our families. However, due to ongoing family health matters and significant challenges in staffing, we have made the difficult decision to permanently close this medical practice. We are no longer providing medical care at 820 W. Lake Mary Blvd; in Sanford.

Over the past several months, many of our patients have successfully transitioned to new primary care providers. If you have not yet established care with a new provider, we strongly encourage you to contact your insurance provider to do so as soon as possible. 

 

For transferring medical records, please have your new provider fax a medical record release to
